Survivor Crisis Fund Application
The Network: Advocating Against Domestic Violence established the Survivor Crisis Fund to address critical gaps in the safety net for survivors of gender-based violence and human trafficking.
Eligibility:
The basic eligibility to receive funding is that you are a victim/survivor of domestic violence, sexual abuse, stalking and/or human trafficking. Beyond that, we have various eligibility guidelines imposed by our funding sources. You will be asked questions on this application to determine your eligibility for all available sources of funding. If you do not qualify for current funding, or if we run out of funds, we may reach out to you if it appears you qualify for future funding.
One-time Funding:
This financial assistance is intended to be a one-time payment out of one of our available funding sources. We will make every effort to help you to receive the maximum available funding option that you are eligible for at the time you apply.
Advocate Assistance:
An advocate can complete the application on a survivor's behalf and the survivor consents for The Network to communicate with them, their advocate/Third Party, or both by providing the contact info for each person. Once the application is submitted, we will reach out to get any clarification or missing documentation. By default, we will communicate with the person who logged into this account to fill out the form.
Privacy:
Your privacy & confidentiality is our concern. Your responses will only be seen by The Network’s team. We will never share your personal information.
Your Safety:
Your safety is also our concern. If, at any point, we do not get a response within two weeks, and we have reached out multiple times, your application will become inactive and we will no longer contact you. You can always reach out to us at SurvivorFund@the-network.org.
Payment of Funds:
If we have determined you are eligible for available funding, we will send you an additional payment details form to complete. Payments are sent as soon as possible once eligibility requirements are confirmed and acceptable documentation is received as long as the funding remains available.
We Cannot Guarantee Funding:
Funding is not guaranteed as our funding is limited. We may close the application portal once a certain number of applications have been received. We will reopen it once new funding is available.
Non-Discrimination:
We do not discriminate on the basis of migration status, race, color, national origin, religion, disability, age, sexual orientation, gender identity, or sex. We will ask optional demographic data at the end of the application form. Reviewers will not see this information.
Questions and Assistance in Completing the Application:
If you have questions or need assistance in completing this application, email SurvivorFund@the-network.org. If you are in need of services from a domestic violence or sexual assault agency, please call the IL Domestic Violence Hotline at 1-877-863-6338.
